Consent & intake, in brief
A full medical and mental health intake, along with informed consent for your tier, is required before your first session. Here's the short version, the complete agreement is emailed to you for review and signature once your deposit is received.
Current medications, allergies, diagnosed physical or mental health conditions, and emergency contact information.
Included only in Awaken and Embody. Participation in any ceremony or protocol is always voluntary, you may decline any specific practice at any time.
Everything you share is held in confidence by your practitioners and never shared outside the Collective without your written permission.
A 50% deposit reserves your enrollment and begins intake. The remaining balance is due by Day 1. Once herbal blends are formulated or practitioner time is reserved on your behalf, payments are non-refundable.

What happens right after I enroll?
You'll receive your Informed Consent & Intake Agreement by email. Once it's signed and returned, we'll schedule your opening intention-setting session and begin coordinating your first month.
Can I switch tiers partway through?
We're happy to customize and add services to your protocol. It's designed to move and change with you, though we do recommend consistency first.
What if I've never worked with plant medicine before?
This is designed to meet you where you are, and support where you're going, at your own pace. Plant medicine comes in many forms; our goal is to find your perfect match.
Is there a limit on spots?
Yes, each cohort is kept small so the Collective can stay close to everyone in it. Enrolling early is the best way to secure your place.
